Our Opportunity: Hospice & Community Care is seeking an Admissions Nurse. The Admissions Nurse will provide care for patients through the admissions process, nursing care to the terminally ill Hospice patient as needed, and to provide assistance and understanding to the family.
What You’ll Do:
Plans for admission of patients in coordination with hospitals, physicians, community agencies, and other referral sources.
Assesses potential patients for hospice criteria. Responsible for establishing customer relationships, collecting pertinent data, and presentation of agency services.
Admits patients, develops initial plan of care in accordance with the patient’s physician, and completes required paperwork.
Makes visits to patients within the service area to provide nursing care, instruction, and support to patients and their family.
Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient.
Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.
As per the South Carolina Nurse Practice Act, Registered Nurses may give medications.
Provides Hospice nursing services, treatments, and preventive procedures.
Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
Attends team conferences, staff meetings, in-services and other agency functions as required.
Maintains records as required by Hospice.
Follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
Maintain skills and knowledge.
Complies with agency infection control policies and protocols.
Adheres to all policies, procedures, and regulations regarding patients care, conduct, safety, fire, security, and risk management.
Assist with orientation, teaching and training as requested. Supervises and trains other nursing service personnel as requested.
Participates in 24-hour on-call nursing coverage when necessary.
Participates in education and community awareness programs as requested.
Other duties as assigned.
What You’ll Need:
Graduate from an accredited school of nursing
Currently licensed as a registered nurse to practice in South Carolina.
Ability to work independently, make accurate, and at times, quick judgments.
Ability to respond appropriately to crisis in a variety of settings. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.
