Community Health Educator/ Peer Adherence Navigator


The Community Health Educator/Peer Adherence Navigator is responsible for the implementation of culturally competent HIV/HCV/STD prevention programs including Counseling, Testing and Referral Services, peer adherence services, interventions and health communication and education for priority populations. This position will provide support from someone living with HIV for others living with HIV. Makes referrals as appropriate. Assists patients in making informed decisions about HIV medication treatment and helps navigate the system of care. Conducts counseling and testing services (both onsite and offsite). Conducts educational presentations with vulnerable populations in conjunction with testing. Participates in Continuous Quality Improvement activities of the organization and Performance Management System.


Requires candidate to be living with HIV.
Good verbal communication skills.
Knowledge of HIV/STD, treatments and substance abuse issues.
Peer Counseling experience preferred.
Experience providing counseling, advocacy, leadership and/or mentoring to others.
Knowledge of resources and familiarity with the system of care.
Organized and dependable.
Knowledge of HIPAA laws, sensitivity to patient confidentiality.
Work independently with minimal supervision.
Strong commitment to teamwork.
