Psychiatric Social Worker Full Time Days


Provides an integrated continuum of care for patients from preadmission to post-discharge.Completes required evaluations.Completes Individualized Treatment Plans.Provides group, individual and family therapies.Screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility.Communicating with patients and their families around plan of care.Communicating and collaborating with physician and other team members.

Minimum Education:Masters degree in Social Work required.

Minimum Experience:1 to 2 years of job-related experience preferred, including experience as a licensed clinical social worker, experience in an acute care setting, knowledge of federal and state laws and CMS requirements and calculations.

Required Certifications/Registrations/Licenses:S.C. licensure in Social Work. BLS upon hire, CPI within orientation period.
